Can I use Cloudflare SSL certificates on my fourth-level subdomain?

The wildcards in SSL certificates match only a single subdomain. The Cloudflare certificates are issued for *.example.com and example.com. This is will match a first level subdomain, such as www.example.com, but not for a second level, such as www.dev.example.com.

On the business or enterprise plans, you can to upload a custom SSL certificate into Cloudflare that matches this hostname.
